As a courtesy to you, we will submit your claim for services to your dental insurance carrier. Please bring your insurance card(s) to your initial appointment. We will assist you in obtaining an estimate of your plan’s benefits for the recommended periodontal/implant treatment.

The final determination of benefits will be decided by your insurance carrier based on your plan’s provisions including the plan maximums. Your estimated financial responsibility is due at the time of the exam/procedure. If your dental plan pays less than expected, we will provide a statement with the balance due. If you do not have insurance coverage, payment in full is due at the time of service.

Please note that we are unable to bill State insurances (Apple Health, Molina, Medicaid, and Medicare).

Under certain circumstances, we are able to make an inquiry with your medical insurance for the possibility of receiving benefits. We would be happy to discuss this with you at the time of your examination.

Preferred Providers

We accept all (private) dental insurances; however, we are preferred providers with the following insurances:

Delta Dental (PPO and Premier Plans)
Principal

*Effective January 1st, 2024, we are no longer considered preferred providers with Regence, Premera, or Cigna insurances.

For your convenience, we accept the following forms of payment: Cash, Check, Visa, MasterCard, Discover Card, and American Express.  Our office also accepts CareCredit, which is a third-party financing company.
